At Your Care Solutions, we provide sensitive, compassionate palliative care that supports you and your loved ones through every stage of the journey. Our goal is to ensure that people can remain in their own homes – where the majority of people say they would choose to be – with the comfort, dignity, and quality of life they deserve.

What is palliative care?

Palliative care focuses on quality of life rather than curative treatment. It is appropriate for anyone living with a life-limiting condition – whether that is cancer, heart failure, advanced dementia, or another serious illness – and can begin at any stage of the illness, not just at the very end of life. Good palliative care addresses physical comfort, emotional wellbeing, and practical needs, as well as supporting family members through a profoundly difficult time.

Staying at home through end of life

Most people, when asked, say they would choose to die at home or in a homely setting rather than in hospital. Yet for many, this wish is not fulfilled due to a lack of adequate support at home. Physical care and comfort

Our care workers are trained in supporting people with complex physical care needs, including assistance with personal care, positioning and pressure area care, managing symptoms under the guidance of the clinical team, and providing general comfort. We work closely with district nurses, GPs, and palliative care specialists to ensure our care is consistent with the wider clinical plan.

Emotional and psychological support

Palliative care is as much about emotional support as it is about physical care. Our care workers provide a calm, compassionate presence – listening, talking, sitting quietly, or simply being there. For many clients and families, the relationship with their care worker becomes one of their most important sources of comfort during this time.

Working with the wider care team

Effective palliative care requires close collaboration between all professionals involved. We work alongside GPs, district nurses, specialist palliative care nurses, and hospice teams to ensure seamless, coordinated support. Our care workers attend care reviews and communicate regularly with the clinical team so that any changes in a person's condition are responded to quickly and appropriately.

Supporting families through bereavement

Our care does not end at the moment of death. We recognise that family members and loved ones need support through the period of grief that follows, and we maintain contact with families after bereavement to check in and offer signposting to specialist bereavement support where it may be helpful. The relationships we build during this time are ones we take seriously for the long term.
